How common is head gasket failure on the 1999 lumina, and at what mileage or warning signal should one pro actively change the gaskets to avoid cylinder head warping or cracking?

What engine? Head gaskets are not a common failure for any engine used in this car. Intake manifold gaskets, yes.
This is not a pro-active replacement part.

It depends on how common it is that you over heat the engine.
