My boyfriend has a '94 Saturn with 216K miles on it. Lately, the car has been idling badly. The car does need a tune up…but what else could it be.
Start with the simple stuff. Change the spark plugs, air filter, pcv valve, and so on. Use a spray cleaner in the intake track (carb/throttle body cleaner). Then, turn it over to a mechanic for some TLC.

You won’t be able to figure out what else it may be until the basic maintenance is done. It needs to be done anyway, so start there. Likely it will not need more.
