That depends on the age of the vehicle, mileage, and a few other factors that may or may not exist.
Careless engine or transmission swap (that seal should always be replaced when an engine or trans is changed), worn out engine (loose main bearings), groove worn into the converter surface, bent or cracked flex plate, or just a plain old aged and dried out seal that decided to give up.

Also, a worn stator bushing which sits behind the seal will cause leakage at the seal. Sometimes, due to poor lubrication, this bushing will walk out of the pump housing and cut into the seal from behind.

If you need to replace this seal it is recommended that the stator bushing also be replaced at this time. I have seen too many times someone replacing the seal only to have to go back in and replace the bushing and the seal. The seal for the second time.
