I haven’t seen many, but I can recall a few jug handle left turns in Maryland.
we have a few on Long Island. never knew it was called that. I just called it an exit ramp.

You have to turn right, then go make a U Turn, it is shown on the bottom of the picture.

In that picture that you shared it seems that there is sufficient room to widen the road to make a left turn lane. Why would they not do this instead?

That would be a question for the traffic engineers.
These replace left turn lanes. They won’t widen the road to add left turn lanes.
Left turns have a high accident rate, so the point is to avoid them.

That is exactly what is happening for the side road. Main Highway left turn is good.
