It’s always used with the ratchet head in the palm so as to provide very little leverage
Exactly the same method I have always used.
Snug with a short 3/8" drive ratchet is good for spark plugs.
…until one of them pops out, like some of the callers from Car Talk have experienced. If I remember correctly, some callers had them pop out with exploding force because the spark plugs worked their way out of the threads.
